Transforming Lives Team (County Wide) – Social Worker Opportunity

We are seeking a passionate and experienced Social Worker to join the Transforming Lives Team, supporting adults with significant care and support needs to be discharged from hospital under the Learning Disability and Autism Programme.

Key Responsibilities:

  • Conduct Care Act assessments, support planning, and risk assessments
  • Commission services and plan safe, person-centred discharges
  • Work creatively with adults to promote independence, wellbeing, and strengths-based practice
  • Collaborate with external agencies in a hybrid working environment (part office, part home-based)

Essential Requirements:

  • Registered with Social Work England
  • In-depth knowledge of the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983
  • Effective communication, negotiation, organisational, and IT skills
  • Ability to work autonomously and manage complex casework
  • Commitment to interprofessional collaboration
  • UK driving licence and access to a vehicle (taxi service available if required due to disability)
