Role Responsibilities

As a Social Worker, your key responsibilities will include:

• Assessing Needs: Conducting thorough assessments to understand the needs and aspirations of individuals and families.
• Creating Care Plans: Developing tailored care plans that promote independence and well-being.
• Collaborating: Working closely with other professionals, families, and community resources to ensure holistic support.
• Advocating: Championing the rights and voices of individuals, ensuring they are heard and respected.
• Monitoring Progress: Regularly reviewing and adjusting care plans to meet changing needs and circumstances.
• Documenting: Keeping accurate records of assessments, plans, and progress to ensure compliance and quality of care.

Required Skills and Qualifications

To thrive in this role, you will need:

• A recognised Social Work qualification (e.g., Degree in Social Work).
• Registration with Social Work England.
• Experience in adult social care, particularly in community settings.
• Knowledge of relevant legislation and best practices in social work.
