An exciting opportunity has arisen within the Younger Adults Reviewing for Independence Team for a Community Care Officer post.

The objectives of the team are to assess and review the needs of younger adults within the County, in line with the Care Act 2014, arrange services to meet eligible needs, and to support individuals to promote their independence and wellbeing within the community. The work is challenging, and strong assessment skills are required. Effective communication skills both with the customers and other professionals is essential. For the right candidate this post offers the opportunity to work alongside all service user groups. The team carry out progression reviews and liaise with the district teams and NES on a regular basis.

In addition to this, the team also undertakes occupational therapy assessments for the Double to Singles team. The team work with customers who have physical disabilities, learning disabilities and mental health needs. Experience in these areas would also be desirable.

We are looking to recruit a committed, well organised and motivated person to join our busy team. The applicant must have at least one year’s experience of working with adults in the community and in a social care or similar setting.

Successful candidates will be provided with a comprehensive induction programme, training, and other developmental opportunities to enable them to complete their role and raise their competency level.
