An exciting opportunity has arisen at Rushcliffe Older Adults Team for a qualified and experienced Social Worker to work primarily with older people and younger adults with Dementia. The role will also involve an integrated approach in working with the Intensive Recovery Intervention Service (IRIS) mental health team, with an expectation that you would base yourself with the IRIS team for part of your working week. You will be experienced in adult social work and be able to demonstrate that they can work in a person centred way whilst adopting the Adult Social Care Strategy with its emphasis on maximising independence, preventing and delaying the need for care and working in collaboration with the voluntary sector and informal carers.

Core tasks will include assessment, support planning and risk enablement alongside utilising the range of resources available within the community to prevent unnecessary admission to hospital and long term care where possible. You will also contribute to duty systems and undertake safeguarding enquiries. We have mobilised our Social Care Teams which means that employees are issued with tablet devices to enable them to work more flexibly and reduce overall travel time. We are currently undergoing a Programme of Transformation to ensure we are a modern workplace and are able to respond quickly to changes within the sector.
