Due to the present post holder gaining a secondment until 31st March 2025 an exciting opportunity has arisen to support our present Reablement Co-ordinators within the Maximising Independence Service Mid Notts Reablement Team (START). This is a challenging and fast paced role, dealing with hospital discharges direct from the Transfer of Care Hub. Good communication and IT skills are a must.
This post is also available as a secondment opportunity to existing Nottinghamshire County Council employees. You must seek approval from your line manager before applying.
