These roles will support the undertaking of the community care assessments and reviews for adults in a variety of settings, including clinics and services users homes with the aim of enabling the individual to maintain and improve their independence.

As a social care assistant you will act as consistent first point of contact for Service Users and colleagues, dealing with incoming queries and supporting them to access services. You will be required to liaise with a number of internal and external agencies and be responsible for the collection of information to complete Service User records.

Part of your role will include booking appointments and arranging clinics, including the collation and provision of information in advance of these. Maintaining team rotas and collating and tracking cases and information on spreadsheets.

Computer literacy is essential to this role as there is a requirement to input and undertake actions on Mosaic.

Successful candidates will be provided with a comprehensive induction programme, training and other developmental opportunities to enable them to complete their role and raise their competency level.

The posts will be primarily office based, working with closely to support locality reviewing teams, based at Lawn View House.
