An exciting opportunity has arisen to recruit a Band A Social Worker; this will include weekend working on a rota basis. We provide regular supervision but in addition have Promoting independence meetings and team meetings with continued professional learning sessions routinely provided for you to participate in.

The post holder will work as part of an established Integrated Discharge Team supporting patients aged 18+ to plan their discharge pathway; this also includes covering accident and emergency. This is an exciting opportunity to be part of an ever changing Integrated Discharge Team aiming to transform current services. We are a team who readily get involved in trialling new ways of working and improving the systems and tools we are provided with to ensure we are working efficiently within the timeframes.

The ultimate aim of the discharge planning is to ensure people remain safe and live independently at home by delivering community based health and social care services thereby preventing further admissions to hospital.

You will be expected to:

  • We keep people at the centre of our agenda and are committed to promoting independence. We work at a fast pace managing a caseload of people living with a range of complex conditions and planning discharges. This work will also include Mental Capacity assessments, Decision Support Tool meetings, Accident and Emergency department discharge planning and safeguarding work.Support managers and the Team to meet the departments’ performance and service delivery requirements, to manage systems effectively and to work positively within a culture of change. The ability to be resilient and effective in supporting staff and managers in the current challenging financial climate.
  • Work autonomously and as part of an Integrated Discharge Team, good working knowledge of the hospital discharge assessment processes and participating in the Integrated Discharge Team’s duty rota on weekly basis.
  • Effective communication, organisational and IT skills are essential. You must also be able to work on your own initiative, prioritising your workload and meet deadlines whilst working under pressure, and have an awareness of community care legislation, policies and procedures particularly around the Care Act 2014.
