We are seeking to appoint 2 Community Care Officers to join the Hospital Assessment Teams at Kings Mill Hospital and Mansfield Community Hospital. We would like to recruit enthusiastic committed, well organised and highly motivated individuals who enjoy working in a busy and fast paced environment. One post will be held at the Mansfield Hospital in Nottinghamshire, and the other will be at the Kings Mill Hospital in Sutton-in-Ashfield.

The posts are within established and supportive teams based on site at these hospitals. The key function of the teams is to undertake Care & Support Assessments with service users and coordinate/commission the services required to enable service users to be discharged from hospital. This includes identifying opportunities to promote independence, for example, using reablement services. There will also be a requirement to complete other assessments as required, for example, Mental Capacity Act Assessments, Best Interests Assessments, and participation in Continuing Healthcare Assessments. The teams have a home first approach and focus on enabling service users to return to their own homes on discharge from hospital using the range of resources available within the community and avoid admissions to long term residential care directly from hospital.

You will need to have an ability to organise and prioritise your workload and be highly motivated to meet hospital discharge timescales, and to respond to changing priorities and demands. There will be a requirement to participate in a duty rota, and, for the Kings Mill Hospital Post this will involve the opportunity to undertake admissions avoidance work within short stay areas of the hospital such as the emergency department.

There will be a requirement to work some weekends on a rota basis as the team undertake seven day working, and there will be enhanced pay for weekend working.
