An exciting opportunity has arisen for a part -time social worker to join the Bassetlaw Hospital Team, which will also cover working at weekends on a rota basis. We work with vulnerable adults with a diverse and complex set of social care and health needs giving the role a huge variety of work to undertake.

The appointed social worker will work in partnership with other professionals and the voluntary sector.

You will be part of the Discharge2Assess Team, ensuring a quick but safe turnaround back home or into care ensuring and maintaining communication with partners to reduce the risk of a readmission back into hospital. Networking with our partners and ensuring a safe discharge plan and everyone is clear on their roles and responsibilities, developing and working in partnership with the voluntary sector and housing.

The intake of work is fast paced to meet the estimated discharge dates and there is a focus on risk management whilst meeting outcomes in line with the adult social care strategy. We are committed to supporting those who are motivated to learn and develop their social work practice. We are a team who readily get involved in trialling new ways of working and improving the systems and tools we are provided with.

We keep people at the centre of our agenda and are committed to promoting independence. There is safeguarding work and systems in place to support the realistic management of work coming through our teams.

Work pivots around the legal framework of the care Act and mental capacity Acts, providing us with clear guidance and supporting our best practice. We use flexible working and use systems to ensure we are always covering the needs of the team as well as providing workers with opportunities to follow work through to the community depending on the discharge pathway. We provide regular supervision but in addition have Promoting independence meetings and Team meetings with continued professional learning sessions routinely provided for you to take part in.
