An exciting opportunity has arisen at Rushcliffe Older Adults Team for an experienced qualified Social Worker to work primarily with older people and younger adults with dementia.
You will be experienced in adult social work and be able to demonstrate that you can work in a person centred way whilst adopting the Adult Social Care Strategy and its emphasis on maximising independence, preventing and delaying the need for care and working in collaboration with the voluntary sector and informal carers.
Core tasks will include assessment, support planning and risk enablement alongside utilising the range of resources available within the community to prevent unnecessary admission to hospital and long term care where possible. You will also contribute to duty systems and undertake safeguarding enquiries.
