Community Care Officer
There is an exciting opportunity for a community care officer to join the energetic, dynamic and thriving team who work across Nottingham University Hospitals and Sub Acute settings
The successful candidate will be part of the Adult Social Care Team who support discharges out of Nottingham University Hospitals and Lings Bar Hospital, completing assessments both in the hospital and in the community. You will undertake assessments/discussions with service users and their families around discharge planning and be required to commission services and monitor packages of care, within set timescales. You must have knowledge of community care legislation, policies and procedures and will contribute to the strategic service development agenda including promoting independence, proportionate assessments and outcome focused personal budgets.

We work closely with health to ensure there are no social care delays out of the hospitals. You will be part of the acute/sub-acute service, which is a dynamic, hard-working, busy and friendly service and will need to be enthusiastic, motivated, and organised in their approach to delivering a quality service. The ability to communicate verbally and in writing with service users, carers and other professional colleagues is essential.
Training and other personal development opportunities are encouraged. This is an excellent opportunity to join a highly motivated service. This post carries a casual user car allowance or taxi service for disabled employees. The ability to work some weekends and on some Bank Holidays is a requirement of this post.
