Nottinghamshire County Council (NCC) Short Term Assessment and Reablement Team (START) provide short-term support to people living in their own homes.

The service operates 365 days a year from 7 am to 10 pm, on a 4 days on/4 days off rolling shift pattern. This includes regular weekend, days, evenings, Bank Holiday and split shift working patterns for our front-line staff. Hours of work are Monday to Thursday 8.30 am -5 pm and Friday 8.30-4.30 pm. There may be a future expectation for Support Co-ordinators to work across seven days a week on varying shift patterns.

This role will be responsible as one of a team of three for the efficient and effective rostering of the START workforce. You will be expected to manage capacity so that the service is able to accept all new referrals into and out of the service in a timely manner with a focus on hospital discharges, preventing hospital delays and community referrals. You will need to work well as part of a team to support this fast moving service.

The post holder will need to have excellent communication and interpersonal skills to enable them to work effectively with a variety of professionals and departments. It is essential to ensure we achieve the best outcomes for the service user whilst operating within strict timescales. IT is an essential element in this role so you will be expected to demonstrate a high level of competence in IT.

Experience of social care is desirable in order to understand the needs of the service. However, significant induction and training will be provided.
