Part Time Social Worker
We are looking for an enthusiastic and highly motivated registered Social Worker to join a forward thinking, supportive team comprising of OT’s and SW’s working with Younger Adults aged under 65. The individuals we support have complex Physical Disabilities and Neurological conditions including for example Multiple Sclerosis, Acquired Brain Injuries, MND and Huntingdon’s Disease.
You will need to demonstrate experience of Needs Assessment and support planning, Mental Capacity Act assessments, partnership work including Continuing Health Care and Safeguarding investigations. In addition to professional experience of working with adults and carers, you will have substantial knowledge of the Care Act (2014) and how Wellbeing and strengths based approaches to promoting independence influences your practice
This is an exciting time to join our team as we are developing a new way of joint assessment and intervention between SW’s and OTs in order to promote individual wellbeing.
