This is an exciting opportunity to join the energetic, dynamic and thriving team at the Queen's Medical Centre.

The successful candidate will be part of the Adult Social Care Team supporting discharges from NUH, and completing assessments both in the hospital and in the community. The successful candidate will undertake assessments with service users and their families. They will be required to commission services and monitor packages of care, within set timescales. They must have knowledge of community care legislation, policies and procedures and safeguarding procedures. 

The successful candidate will contribute to the strategic service development agenda including promoting independence, proportionate assessments and outcome focused personal budgets.  Our philosophy is 'Home First' and the successful candidate will need to show how they can work within this. We work closely with health services to ensure there are no social care delays out of the hospital. They will be part of the hospital based social work team, which is dynamic, hard-working, busy and friendly. They will need to be enthusiastic, motivated, and organised in their approach to delivering a quality service. The ability to communicate verbally and in writing with service users, carers and other professional colleagues is essential. Training and other personal development opportunities are encouraged.

This post carries a casual user car allowance or taxi service for disabled employees.

Ability to work some weekends and on some bank holidays is a requirement of this post.
