We are looking for a Community Care Officer to join the Carers Team at the Adult Access Service based at the Piazza, Annesley. You would be joining a dedicated group of individuals with a vast amount of knowledge and experience. You would be responsible for undertaking Carers’ Assessments and Reviews and Support Plans with appropriate information, advice and commissioning of Personal Budgets, NHS Carer Breaks, Emergency Cards, etc. Ensuring carers receive accurate and timely information / advice e.g. universal services available such as the carers’ emergency card and the Carers’ Hub. Developing up-to-date knowledge of services supporting carers in all districts of the county. Providing a duty point, offering advice and information regarding carer support.   

We are office based and the work is completed over the telephone. You would require good communication and strong IT skills. We will offer you regular supervision, support and the opportunity for training and personal development.
