The successful candidate will be required to work 37.5 hours a week - flexible working patterns will be considered.
Bassetlaw Community and Voluntary Service are looking to recruit a Social Prescribing Link Worker who will be based within the Primary Care Network Newgate Medical Centre. The Primary Care Networks are designed to link staff from general practice with other local services to deliver joined-up care for the local population.

In addition, you will be required to forge strong links with local voluntary and community sector organisations, providing a two-way link between them and Bassetlaw GP Practices and statutory services. A key part of this role will be to develop new support groups and strengthen community resilience. You will work in partnership to proactively raise awareness of social prescribing and its benefits. Ideally, you will have a strong knowledge of the Bassetlaw District.
